I've been plagued by this problem for years, both before MS and now with it.
1. First, investigate whether the duplicates are the same category. For instance if one of them is always the right category but the other is always "imported" or "unfiled" then just go in and delete everything in that category.
2. If they are genuine duplicates, run the wonderful little program "UnDupe" on your Treo (http://www.stevenscreek.com) then sync again. Let the desktop have plenty of time afterwards to digest the new changes (it has internal syncing with iSync that must occur) then sync again. Usually a few rounds of this is enough to stamp out all or most of the problem.
Good luck! Let me know how it works. If you ever discover the root cause please e-mail me since I have some variation of this problem (usually partial database duplication) every few weeks.
