I also had this exact problem when I was using 4.0.2's beta version. Spent a
long series of exchanges with support but never fixed the problem. Recently
after 4.0.2's update, all problems disappeared. No ideas why.

I would suggest that you use OnyX and try to clear some of OS's caches (read
instruction very carefully). Maybe try another installation. Temporally in
my case, the clearing of cache may have made a difference, but I have no
absolute proof.
